This contract will allow you to sign up
for DSL service with Pipeline-DSL.com on an annual basis, in return for which
you will receive a modem/router for use during your service period.
The pricing and terms and conditions are
on our website at www.pipeline-dsl.com. Before you sign this form please read
those prices and terms and conditions located on our website.

indicate that you wish to sign up for a minimum one year commitment on your
account in order to get modem for no charge.

indicate your understanding that the modem remains the property of Pipeline-DSL.com. If you close your account at any time,
even after one year, you will need to return the modem to us, or pay us $299 to
replace it.
_____Initial here to indicate that a one
year commitment means even if you move or quit for any reason, you still have
to pay for one year of service. If you are paying by credit card, this authorizes
us to make that charge.

would like use to host a small website and email for an additional $25.00 per
month. We hope this
